Retatrutide + Cagrilintide
What it is
The most ambitious combination in the catalog: the compound that copies three hormones at once —the most potent published— together with the amylin analog. Four different satiety and energy expenditure signals in a single vial.
Retatrutide already presses three keys: two that curb hunger and one that increases energy burn. Cagrilintide adds a fourth, amylin, which is independent of the others and prolongs the feeling of fullness. The idea is to cover energy balance from both sides at once: what comes in and what is spent.
What it is used for
- • Research on weight control combining incretins and amylin
- • Energy expenditure and thermogenesis, the branch contributed by glucagon
- • Satiety targeted through four different signals
- • Body recomposition and fat mass
How it is used
Lyophilized powder with both peptides combined. It is reconstituted with bacteriostatic water, swirled without shaking, and stored between 2 and 8 °C. In the literature the components are administered weekly, always with gradual dose escalation [1][2].
What has been observed
Results described in the published scientific literature. They are not directions for use or health recommendations.
- • Retatrutide achieved a mean weight reduction of 24.2% at 48 weeks in its phase 2 trial, the highest figure published for an incretin agonist [1]
- • Cagrilintide reached 10.8% as monotherapy in its phase 2, and outperformed daily liraglutide despite being injected once a week [2]
- • That amylin and incretins add up is not a hypothesis: the combination of cagrilintide with semaglutide reached 20.4% in phase 3, above each component separately [3]
- • For both components, the adverse effects described are mostly digestive, dose-dependent and can be attenuated with slow escalation [1][2]
How it works
Retatrutide — GIP / GLP-1 / glucagon triple agonist
It acts simultaneously on all three receptors [1]. The GIP and GLP-1 branches reduce intake; the glucagon branch is associated with greater energy expenditure and mobilization of liver fat, and it is what distinguishes it from dual agonists [5].
Cagrilintide — amylin analog
A long-acting analog designed to resist degradation [4]. It acts on receptors in the area postrema and slows gastric emptying, a pathway independent of incretins [2].
The rationale for the combination
Four targets spread across two systems that do not overlap. The precedent of cagrilintide with semaglutide shows that adding amylin to an incretin agonist produces a greater effect than the incretin agonist alone [3]; this formulation applies the same rationale to a more potent agonist.
State of the evidence
Solid for each component, nonexistent for the combination. Each of the two peptides has published randomized clinical trials [1][2], and the principle of combining amylin with an incretin agonist has been validated in phase 3 with another pair [3]. But there is no published study of this specific formulation: it is research material, not a medicine, and its combined profile has not been characterized.
