Summary
IGF-1 LR3 (Long R3 IGF-1) is the long-acting variant of insulin-like growth factor 1, studied for its role in muscle hyperplasia, recovery, and nutrient transport. Technically, it is an 83-amino acid analog with an arginine substitution at position 3 and a 13-residue peptide extension at the N-terminal end, which drastically reduces its binding to transport proteins (IGFBPs), prolonging its half-life and biological activity compared to native IGF-1.

Mechanism of action
IGF-1R Receptor Agonism
It activates the IGF-1 receptor (IGF-1R) and triggers the PI3K/Akt/mTOR and MAPK/ERK pathways, central routes for protein synthesis, proliferation, and cell survival.
Prolonged half-life due to low affinity for IGFBP
The N-terminal extension and the Arg3 substitution reduce its capture by circulating IGFBPs, so a larger fraction remains free and active, multiplying its potency compared to recombinant IGF-1.
